Quoted on 2026-08-07 out of Berkeley Heights, NJ. The refusals are as useful as the prices, because they say where the parcel network ends.
Six devices, one quote run
| What we typed | Packed box | Going to | Cheapest quote |
|---|---|---|---|
| ResMed AirSense 11 CPAP machine with humidifier in a travel case | 14 × 12 × 8 in 8 lb | Atlanta, GA | $15.64 FedEx Ground Economy, 5-6 business days |
| Inogen One G5 portable oxygen concentrator with built in lithium ion battery | 13 × 11 × 9 in 9 lb | Chicago, IL | $15.73 USPS Ground Advantage, 3-4 business days |
| Tabletop nebulizer compressor | 11 × 9 × 7 in 5 lb | Phoenix, AZ | $19.14 UPS Ground Saver, about 5 business days |
| Folding rollator walker with seat | 32 × 26 × 12 in 22 lb | Dallas, TX | $81.29 UPS Ground, 5-6 business days |
| Four wheel mobility scooter with sealed lead acid battery | 48 × 25 × 36 in 120 lb | Miami, FL | No parcel option came back |
| Electric hospital bed frame | 88 × 38 × 16 in 180 lb | Chicago, IL | No parcel option came back |
Two rows repay a second look. The sleep machine returned four options inside sixteen cents of each other, and the one in the middle at $15.73 carried a 3-4 business day estimate against 5-6 for the cheapest — the days are the variable there, not the money. The rollator is the opposite lesson: 22 lb on the scale, nearly ten thousand cubic inches of box, and $81.29 to Texas. Folded walking frames are mostly air, and air is what the price is for.
The battery decides which services you get to see
The concentrator and the sleep machine are almost the same box. The sleep machine's board came back five lines long; the concentrator's came back three, and every one of the three was a ground service. That is the battery talking. A device quoted with a lithium cell installed is offered the ground network, and the expedited lines drop off the board before you see them.
The postal side of that is written down in Publication 52, section 349, read on 2026-08-07. It sets the ceilings for what may be mailed: “Cells must not exceed a 20 Wh rating” and “Batteries must not exceed a 100 Wh rating”, with a limit of “no more than eight cells or two batteries” in a mailpiece. Loose cells are treated more strictly than installed ones — 349.244 requires individual batteries be “sent by surface transportation only”, with a narrow exception for air inside Alaska. And 349.6 puts “Damaged, defective, or recalled batteries” outside the mail entirely unless approved by the director, Product Classification.
Leave it in the device
A concentrator, a portable suction unit or a battery-backed pump is designed around its own pack. Installed, it is one article with one set of rules. Pulled out and dropped in the same carton, it becomes loose cells travelling next to equipment, which is the harder case.
Spares are a separate decision
The second battery a patient uses on a long day is the one that turns a simple parcel into a restricted one. Send it on its own quote, or leave it behind and let the recipient buy locally.
A swollen pack does not travel
Bulging cases, a pack that has been recalled, or one that came out of a dropped device: those are not shipping problems to solve, they are batteries to take to a recycler. The postal rule names damaged, defective and recalled batteries together.
On this site the same logic runs at checkout. Describe the item as a device with the battery installed and the quote comes back with ground services and a short confirmation step; leave the battery out of the description and you get a service list the parcel would not survive.
The medical items a quote will not price at all
UPS publishes a plain-language list of common items that may be hazardous, and the medical entries on it are short enough to read in a minute. All quoted below were read 2026-08-07.
| Entry | How UPS describes it | What it catches |
|---|---|---|
| Breathing Apparatus | “May mean oxygen cylinders under pressure” | Any tank of medical oxygen, full or nominally empty, and the regulators sold with one |
| Chemical Oxygen | “May indicate the presence of an oxygen generator” | Emergency oxygen canisters that make gas by chemical reaction rather than storing it |
| Lead-Acid Batteries | “Common in cars, electric wheelchairs, some continuous computer power sources” | Power chairs and mobility scooters, whose packs stay with the frame |
| Medical Supplies | “May contain flammable or corrosive chemicals, infectious substances or radioactive” | Anything clinical rather than domestic: sharps, cultures, reagents, isotopes |
| Dental Apparatus/Equipment | “May contain hazardous chemicals such as resins, solvents, or gases” | Chairs and handpieces sold on second-hand, still wet with their own consumables |
| Thermometers and Barometers | “May indicate the presence of mercury” | Old glass thermometers and sphygmomanometers out of a family cupboard |
One distinction saves a lot of confusion. A portable concentrator is a pump: it pulls room air through a sieve bed and hands back a richer stream, storing nothing under pressure. The regulated articles on the list above are the cylinder and the chemical generator, not the machine that makes gas as it goes. That is why our concentrator quote returned rates while a cylinder would not have.
Where a device stops being a parcel
The two empty rows in the first table were predictable from limits UPS publishes on its own dimensions and weight page: a maximum package weight of 150 lbs, a maximum length of 108 inches, and 165 inches of length and girth combined. The page adds the consequence in its own words — “Packages above certain size and weight combinations may require freight shipping services”. Read 2026-08-07.
Run the two refusals through that. The scooter crate at 48 by 25 by 36 inches measures 48 plus twice 25 plus twice 36, which is 170 inches of length and girth — past the ceiling on size alone, before the lead-acid pack is even considered. The hospital bed frame is over on both counts: 180 lb against a 150 lb limit, and 196 inches of length and girth. Neither is unshippable. Both are freight.
Beds, patient lifts, exam tables, treatment chairs and power chairs travel on a pallet or a lift-gate truck, priced by class and dimensions rather than from a parcel table, and delivered to a room rather than a doorstep when somebody books that. SMKlog sells parcel labels only and does not arrange pallet or freight shipments. If you are weighing the two routes, parcel against LTL sets out where the crossover really sits.
Packing a used device so it works on arrival
- Empty and dry the humidifier chamber. A tablespoon of water left in a sleep machine finds the electronics somewhere around Pennsylvania, and it is the single most common way a working unit arrives dead.
- Bag the mask, hose and filters separately. A loose length of tubing whipping around inside the carton is what cracks a housing.
- Use the original carton when you still have it. These devices ship from the factory in molded pulp that fits them exactly, and nothing you build at home holds a motor as well.
- Tape over any switch that a box wall can press, and coil the cord with the plug turned inward so the prongs cannot dig into the case.
- Fold a rollator, strap the frame closed and pad the wheel forks. Bare forks punch through single-wall corrugate from the inside.
- Photograph the serial plate before the lid goes on. Second-hand medical gear is bought and sold on serial numbers, and the photograph is also your evidence if something goes wrong on the way.
- Describe the contents the way they are. A device with a battery installed, said plainly, produces the service list that parcel can actually carry.
Where these rules come from
- USPS, Publication 52, section 349 — the 20 Wh cell and 100 Wh battery ceilings, the eight cells or two batteries per mailpiece limit, surface-only carriage for individual batteries, and the bar on damaged, defective or recalled batteries. Read 2026-08-07.
- UPS, common items that may be hazardous — the breathing apparatus, chemical oxygen, lead-acid battery, medical supplies, dental equipment and mercury entries quoted above. Read 2026-08-07.
- UPS, shipping dimensions and weight — the 150 lb, 108 inch and 165 inch package ceilings, and the note that larger combinations may need freight. Read 2026-08-07.
Carrier limits and postal battery rules are revised on their own schedules, so treat the figures above as what those pages published on the date shown. The prices in the first table are live quotes from the same day for those exact boxes and lanes, not a rate card — re-quote the device in front of you.
Common questions
How much does it cost to ship medical equipment?
$15.64 to $81.29 for the four home devices priced here on 2026-08-07, all leaving Berkeley Heights, NJ. The two larger items in the same run, a mobility scooter and a hospital bed frame, returned no parcel option at all and belong on a pallet.
Can a CPAP machine be sent by parcel?
Yes, as an ordinary parcel. Ours quoted at $15.64 to Atlanta in a 14 by 12 by 8 inch box at 8 lb on 2026-08-07, with four services inside sixteen cents of each other and transit estimates spread across two days. Empty the humidifier chamber first.
Does a portable oxygen concentrator ship like an oxygen cylinder?
No, and the difference matters. A concentrator holds no gas under pressure, so it quotes as a device with a lithium battery installed and moves on ground services. UPS lists breathing apparatus meaning oxygen cylinders under pressure, and chemical oxygen generators, among items that may be hazardous.
What lithium battery limits apply to a mailed device?
Publication 52 section 349 sets a 20 Wh limit on a cell and 100 Wh on a battery, allows no more than eight cells or two batteries in one mailpiece, and requires individual batteries to travel by surface transportation only. Damaged, defective and recalled batteries are excluded.
When does medical equipment have to move as freight?
Once it passes the published package ceilings. UPS caps a package at 150 lbs, 108 inches of length and 165 inches of length and girth combined. Our scooter crate measured 170 inches and the hospital bed frame 196 inches at 180 lb, and neither returned a parcel option.
